The National Diabetes Prevention Program (DPP) is a CDC-recognized evidence-based lifestyle change program that can help you prevent or delay type 2 diabetes. Research shows that participants who make modest lifestyle changes can reduce their risk of developing type 2 diabetes by 58% - and by 71% for people over 60.

Join a supportive group of peers on the same journey. Our trained lifestyle coaches guide you through a year-long program focused on healthy eating physical activity and behavior change - without extreme diets or intense exercise.

Program Structure

A Journey to Better Health

  • Months 1-6: Weekly sessions focusing on building healthy habits
  • Months 7-12: Monthly sessions to maintain progress and prevent relapse
  • Format: Small groups of 10-15 participants
  • Options: In-person or virtual sessions available

Program Goals

  1. Lose 5-7% of body weight
  2. Get at least 150 minutes of physical activity per week

Am I Eligible?

You may be eligible if you:

  • Are 18 years or older
  • Have a BMI ≥ 25 (≥ 23 if Asian)
  • Have pre-diabetes or high risk for type 2 diabetes
  • Are NOT currently pregnant
  • Do NOT have type 1 or type 2 diabetes

Coverage: Covered by Medicare for eligible beneficiaries. Many insurance plans also provide coverage.
