The National Diabetes Prevention Program (DPP) is a CDC-recognized evidence-based lifestyle change program that can help you prevent or delay type 2 diabetes. Research shows that participants who make modest lifestyle changes can reduce their risk of developing type 2 diabetes by 58% - and by 71% for people over 60.
Join a supportive group of peers on the same journey. Our trained lifestyle coaches guide you through a year-long program focused on healthy eating physical activity and behavior change - without extreme diets or intense exercise.

A Journey to Better Health
- Months 1-6: Weekly sessions focusing on building healthy habits
- Months 7-12: Monthly sessions to maintain progress and prevent relapse
- Format: Small groups of 10-15 participants

Am I Eligible?
You may be eligible if you:
- Are 18 years or older
- Have a BMI ≥ 25 (≥ 23 if Asian)
- Have pre-diabetes or high risk for type 2 diabetes
- Are NOT currently pregnant
- Do NOT have type 1 or type 2 diabetes
Coverage: Covered by Medicare for eligible beneficiaries. Many insurance plans also provide coverage.
